Pretty Photo for Pop Up Form using Gravity Form

I’d like to have a button, or an image for that matter, that would activate the Pretty Photo Pop Up. From what I undertand of this, I just need to add the relative=”prettyPhoto”. Here’s the thing though, I would like this pop up to contain a form, not the image or button used.

So -> Button -> Pop up -> Form

How can I achieve this? Thanks
