There are a few approaches to this.
You could either use the contact.php template as a starting point for creating your new template that you can apply to your new page by editing it and leaving whatever you want from the default one in, or you could install a form creating plugin like “contactform7″ and use that, I suggest you go for a plugin as it will offer more flexibility for your form, you can add/remove fields, change labels etc. Good luck! :)
