Contact Form Broken

Hi there, I am having problems with the contact form on my site, when someone tries to send a message they get this error:
“Failed to send your message. Please try later or contact the administrator by another method.”
Why would this be? Thanks

I’ve actually fixed it but I don’t think I’ve done it right (I’ve put the same email address in the ‘to’ and ‘from’ fields – is this correct?

There also seems to be a ‘syntax error’ in the form itself, looks like it’s something to do with the ‘subject’ box as it is highlighted red in the mail tab and there is a warning icon on the ‘form’ tab?

Do you get mails from it?
You can inspect all tabs of your form to see if all field names are matching…

I think I have fixed it now (fingers crossed) thank you one thing I am unsure of, in the ‘mail’ tab, is it ok if the ‘To’ and ‘From’ boxes have the same email address in them (I only have the one)? Thanks
