Somehow the ajax call breaks, here’s the errors I got too:

ot-admin.js?ver=2.0.12:463 HEAD http://www.maaspublications.net/testsite/FEE/wp-admin/undefined 404 (Not Found)

This is very strange as I have never encountered an issue like this before, I also tested with a mirror version of your WP and theme version on our testing server, the logo setup works fine.!

Has anything changed on your server settings and has this feature ever worked there?

I am afraid the issue is deeper than what I thought it was.
I ran some tests and the PHP version has to do with it too, on the older PHP version 5.4 the issue isn’t present, and on PHP 5.6 it appears.
Will update the version of the Option tree plugin soon, that should fix it.
