I need some help.

I created a custom form inside a RAW HTML container, at the beginning I call a Java Script saved into the /wp-includes folder. This action modifies the page layout.

The Submenus are not showing on this page and also the top Full_Header div is not showing.

Need some help to solve this issue, i have tried a 1,000 things and nothing seems to solve the problem.

http://es.flexopluss.com/solicitud-de-cotizaciontf/ is the page that show the issue.

On the page menu: Under COTIZACIONES -> Troquel Flexible.
